Requirements

To file a probate, you must provide:

• An Application for Informal Probate
• Original Death Certificate
• Last Will and Testament (if decedent had one)

*When printing probate documents from the supreme court website, please only print single sided.

Decedent’s death must not have occurred less than 120 hours and no more than 3 years from the time of filing.
